Display Request Description in Associated Task

I found an article detailing some steps to make the request description display in an associated task:

I've done my best to work through the steps, and when I click save and test in my custom function, I get a success message, but still the request description is not populating in the associated task. I am not 100% sure I have the Custom Trigger configured correctly, the instructions were not easy to follow for that step.

Any ideas? Anyone willing to share screenshots of a functional custom function/custom trigger?
