User Management Service Catalog Issues

I have been tasked with re-vamping our new hire forms now that we have MESD+.  I found the Service Catalog and was very grateful to find the User Management section.  I immediately found the new hire form and edited it to fit our specific needs.

I found the workflow and added an approver and feel like I'm on the right track now but I have an issue.  We want tickets or tasks to open once the request has been approved based on the results of certain fields in the form.  

We're a SOX compliant company so we have certain procedures to go through just to install some of the software we use.  I want for someone to select the software they want for the new user (that part works) and then I want certain tickets or tasks to open based on what they have selected.

I found form and field rules and found the section where I can make tasks based on the value of certain fields once the form is submitted but the only fields I can select from that list are in the requester details section, not in the resource info section which is where I added all the questions and fields that I need to trigger the tasks/tickets.  

I also see the ability to add tasks on approval of the ticket from the workflow page but I don't see a way to attach those tasks to a certain value on a field on the form.

How can I do this?
