Request Trend Report

Hi:
In need of a report that shows Request trends by month (Jan,Feb, Mar,April,....
That is, for each month we would like to see:

• Number of Tickets Opened
• Number of Tickets Close
• Number of Tickets Ohhold

If possible can a Line Chart be included (see attached)?
