Open Requests Create new Requests Every month

I have 4 open requests for an ongoing project. Every 1st of the month these requests re-create themselves. it has happened the last 2 months. So now i have a total of 12 requests open for the same project. I've looked around and cannot see what is governing this.

Has anyone had this issue?
