Incident Template - Requester unable to input data in Contact Number, Job Title, etc...

Incident Template - Requester unable to input data in Contact Number, Job Title, etc...

How to I set these fields to where a requester can input their data.  There is no drop down option to select "Requester can set" that I can find.  They have asked to be able to add their details in the Contact Number, Job Title and Department fields.


Any help would be gratefully appreciated.
