Icon in survey webpage

Hello,

We use SupportCenter Plus 7.9.0 Build 7935. We recently activated the survey feature which works pretty nice. We would like to change only one thing:

We would like to add an icon to the survey webpage. Could you tell me where to find the HTML file that I would have to alter? Or is there another way?
