Help with user creation

I was trying to create a user, using a template I put together and when I went to creaet it I got this message back in my browser;

Anyone know where I should start troubleshooting?

Should ADManage be installed on the domain controller to allow it local control, as I'm using it on a different server currently with an mmc snapin.
