In many cases, IT technicians send usernames and passwords of some software services in response to user tickets.
These details are communicated to the users both in the service desk and in the email, and the passwords can be seen in plain text in the description of their request! That this is an information security leak.
I suggest that a password field be added to the access request forms so that it is shown as a star in the email also the user in the self-service portal can view the password via clicking on view Password icon.
