Using DUO when Unlocking Account/Resetting password

I've enabled DUO security as per the documentation ( https://www.manageengine.com/products/self-service-password/kb/duo-enabled-two-factor-authentication.html) but when I try the unload/password reset feature I never get a DUO notification!

I verified that the server can access the "API hostname", however we do have local DUO proxy server.

Any idea where I might look for a solution?
