Unable to reset password - fails for all users

The Unlock your Account feature works beautifully, but none of our users are able to use the Password Reset feature. User receives the following error after attempting to reset their password. 

"Reset Password Failed - 1. The new password you chose failed to meet any or all of the standards set by your administrator (as given below): 2. Minimum Password Age: If this is set, you cannot change password for a specified time."

Here's a sample scenario. Bob is an enrolled user and forgets his password. From the GINA interface, he selects Reset Password, answers his security questions correctly, selects a new password that meets all security requirements and submits his selection. The above message displays and now Bob is unable to login with his "new" password or his old one. Helpdesk must then force Bob to change his password from within Active Directory.

It's almost as if the passwords are getting corrupted between the user submitting his request and active directory. I've checked all that I can think of without success. We're on version 4.4 build 4453.
