Unable to fetch values from ServiceDesk Plus

I am testing OpManager and Service Desk Pro and followed the instruction below to integrate OpManger with Service Desk Plus, without error (i.e. Successfully tested and added the ServiceDesk Plus server details)

Opmanager (v 12100 (64 Bit)) 
 ServiceDesk Plus (v9.2 Build 9227) 

However, when I added a new notification profile (i.e. click Log a Ticket under Add New), " Unable to fetch values from ServiceDesk Plus" is shown.  I am using the SDP administrator account under Authentication Details in OpManager (Add-On/Product Integration). 

Please advise what's missing or unable to fetch in order to proceed. Attached the SDP and OM logs (time: between  12/09/2016 10:29 AM and  12/09/2016 11:29 AM).
