Unable to add device - Not in inventory or search

I'm trying to add a device to OpManager. I can add it through Settings > Discovery > Add Device, but this does not allow me to attach rules to the discovery.

When attempting to add via Settings > Discovery > Discovery > IP, I can associate rules, but the device is never added to the inventory. I've searched through the business views that it SHOULD have been associated with, as well as searching the inventory, but the device is never added.

Please advise
