Surveys do not work in Servicedesk plus Enterprise

We are using build 7009 of the Enterprise version of Servicedesk Plus.

The surveys have never worked for us. The email gets sent out as expected and the user receives the survey. There is no space between the 'Welcome' and the person's name which looks un-professional.

However, worse is to come. When the survey is filled in and the submit button is clicked we get the screen that I have attached to this post. As you can see 'Welcomenull' is at the top, which again is un-professional and what is worse is that no success/thanks/fail message is generated. Also, the survey is not sent back to the Results section of Servicedesk so we cannot keep track of the responses. This screen stays put even if you click submit again.

Is it just me or should these issues not have been very noticeable in AdventNet's testing phase?! I reported this issue at least two hotfixes ago and it still has not been resolved. I would appreciate a response as to when this will be fixed.
