The survey link is now broken ( http://servicedesk.horizons.govt.nz/sd/SurveyDetails.sd?surveyData=7d6499308bd38b65cc82419b4a4e32bba643af17).  The rest of SD+ behaves as expected.  The Server has also been restarted.  Any suggestions on where to start looking?
