Successful Upgrade from 4112 to 5100... but

Firstly, many thanks for the release of the upgrade: I have applied to my Test server but;
The MySql migration log throws some really scary messages around unique constraint violations and not null columns containing nulls but appears successful ?!? Has anyone else encountered the same issue?

Also, can we lose (or at least make optional) the mandatory "Quick Create" section on the Requestor Home Page? I have gone to great efforts to customise a decent form, so the last thing I want are lazy users filling out the bare minimum to satisfy a Quick Create... (see attachment QuickCreate).

Why can't I add a file attachment to the Resolution of a Request?

Is is possible to tidy up the rendering of the Customisable Request Form: Indentation of Customised Fields is clumsy (see attachment Form).

Subject to successful acceptance testing, I hope to implement into PROD early January.
