It would be useful for us to track barriers, issues and decisions holding up projects, as well as risks as in a risk register.

As a workaround, we created two task types Issues and Decisions using comments primarily for Issue resolution and and point to steering committee meeting minutes or other documentation for Decisions.

How are others handling Issues, Decisions, general barrier tracking and Risks?
