This has to be done before but I'm not sure what to search for. 

Found some examples but not that match my case. 

I'm going to setup a service catalog Service where the customers can request a new user. 

I have a base template but it's only for IT access now the customer has requested to have option to request access to facilitets and other services outside the IT department.  My idea was to use check boxes to hide/unhide fields based on what they need( Tell me if there are another easier way).

But I need send a e-mail with the info needed for each service. I don't what to send all information as some are sensitive information.   It's where I'm stuck. 

My idea was to use the On Form submit, Something like this;  If  checked box  "Facilitet" = set  send e-mail to "facilitet e-mail adress" with Facility fields information.
