SDP Service Catalog - Workflow

Hi all,
I am testing the Service Catalog feature right now and came to an issue or question on the workflow. In my company the process usually starts in the HR dept. So they should start with the service request for new hire and fill in only few information such as employee name, work start date, department and job title. Next what should happen is that the head of department is notified about the request and he/she need to fill in additional information about the needed equipment, work place and so on.

How do I create such a request form? Should I create all the needed fields and add and use rules to remove/hide on load if the user is member of the HR dept but add/show when the user is a head of department or what is the best option here?

Also how do I trigger to notify the head of department when the request have been created to complete the request?
