I have a Subject update script I use on one of my forms. It works just fine. I have another template I've built and I'd like to use the script on it as well. The script just pulls information from fields on the form and adds them to the subject.

I've updated the script accordingly to match the new field I want it to pull from. The field and form rule is the same as the one that works as well. But I cannot seem to get it to trigger. I've attached a picture of the working script and the new one that isn't working, as well as the field and form rule I'm using.

Any help to get this working would be appreciated!
