We have a SharePoint modern intranet site we use for colleagues to access the document library, company news, events, to complete forms and links to software applications.

I know the requester portal exists in SDP but how can I embed the requester portal, or at least give colleagues the ability to log an incident, from within our SharePoint site - without the user having to leave the intranet to open up, and log in to, the SDP requester portal?

Ideally they would see the incident template fields for them to complete within the SharePoint page. 

Otherwise, it seems as though I need to create a SharePoint form that has the same fields as the SDP incident template and when it is filled out it emails the SDP mailbox to generate a ticket that way - which seems a bit clunky?
