Request Form Fields

Hello:

First of all thanks for latest update, Alphabetical sorting works fine.

I have few other issues, though:

1) When I go into admin page for request form preview, it does show all correct fields (snapshot attached), but few fields like Technician, contact #, Department, Level are not seen at all when requester goes to create web form request. (snapshot attached)

Let me know if I am missing something and how to fix this.

2) We have our corporate Intranet and would like to link ServiceDesk HelpDesk feature in such a way such that once user logs in it directly goes to new request form, i.e we would like to link as below

http://server:8080/WorkOrder.do

Right now when I test this it comes up with broken request form (snapshot attached) is there any way to fix this, it might something on HTML coding side. When I do refresh on that page it does show whole request form. Any ideas?
