I am looking for a report that will combine the Service Request with a Change Request that was spawned from it.  I need to be able to show the following fields:

 

Service Request Number

Category

Subcategory

Requestor

Subject

Descritpion

Techinican

Change Request Number

Change Type

 

I will need to be able to filter on Category and Change Type.  Would you be able to help?
