Query Report for Non-Close/Cancelled/Resolved Tickets Greater Than 5 days with Specific Value in 'Incident Additional Field'

I'll looking to create a query report for tickets which are not close, cancelled or resolved, which are older than 5 days with a specific value in the incident additional field.
