Problems with GINA

I have installed ADSSP 4.5 and verified installation of my license. I have added seven domains and successfully sync'd all seven domains with a custom service account with domain admin rights in all domains. I am able to parse the directory structures through ADSS, and I can ping all end-points that I'm attempting to deploy GINA to in each domain.

I am running ADSS in application mode (not as a service) and the database is hosted on a a SQL Server 2012 Enterprise server.

PROBLEM: I cannot get the GINA to deploy, I get various errors from "network path not found", to couldn't "connect to ADMIN$", "Couldn't start remote service. Access is denied", to "network path not found/Invalid credentials" To the mother of all errors...

This error pops for as many installs as I've set to attempt.. Pop up after pop up after pop up. I set it to always run as administrator just to see if that changed the behavior but it did not.

NOTE: I have attempted deployment of GINA to server 2008, Windows XP Pro, Windows 7 Pro and server 2003 all failed, over 200 machines regardless of whether one machine or all machines were selected for install.

Finally I was able to deploy the GINA to one Windows Server 2008 R2 Datacenter edition once when I had it in service mode (not application mode) but I could not repeat the success even on the same machine, and the tech support rep had me change it back to application mode after that.

Here's what I need from anyone willing to help me solve this:

Clear instructions on any prerequisite services or settings on client and server machines to get GINA to properly deploy.
Clarification on services logon rules when ADSSP is in service mode.
Tips on how to troubleshoot errors in greater depth and detail than the support tech and the help documentation can apparently offer.
Anything else I don't know to ask for when getting GINA to deploy, update, and customize in a complex multi-domain environment.
