Problem when adding a new request template

Problem when adding a new request template

A fix was applied by the support team last week which cured a problem with additiona lfields that had been added.
 
However when I now try and create a brand new template, i.e. clicking on Save, the screen reports:
 
'Processing. Please Wait....'
 
and simply remains like that indefinetely.
 
Please help.
 
Actually - I've now received a message stating 'Request Template Save Failed'
