I would like some help generating a monthly report to be processed in an external application.

I need a listing containing the following information:

Computer name, Domain, Operating system, Patch ID, Patch description, Severity, Deployed date, Remarks, Deployment status, Patch status, Patch type, Vendor.

For this, I need to know which database tables I can use to obtain this information.
