OpManager to ServiceDesk Plus Cloud Integration

OpManager to ServiceDesk Plus Cloud Integration

We have not been able to log a ticket from OpManager to SDplus Cloud.  The integration seems to be setup as required.  The sync button works and says it has synced successfully.  When I try to test the log a ticket function using the "Test" button it returns the following error:

Failed to add request in ServiceDesk Plus Cloud.
400 {"response_status":{"status_code":4000,"messages":[{"status_code":4012,"type":"failed","fields":["mode","level"]}],"status":"failed"}}
