OpManager NOT sending SMS/Email notification on "Critical" state of devices.

I normally receive emails and sms's when any of my devices meets the following criteria:

when the Device misses   poll(s)
when any [selected...] Interface is having some problems
 
Select all

when any [selected...] SNMP trap is received from the device.
when any [selected...] Threshold rule is violated
when any [selected...] Syslog Rules generates alarm
when any [selected...] Virtual Device has problem
notify when the alarm is cleared

Critical Trouble
Attention Service down
However, regardless of the above rule set in place for a specific Router, I was not notified through SMS or Email when this device went of today at  Jan 06,2012 08:21:56 AM

I also receive emails from Anti-Virus Server, every morning, same subnet where OpManager server reside, so this confirms that it is not my ISP or my mobile handset.

What log's do you require so we can start troubleshooting?
