Opening a ServiceDesk Ticke with OpManager Premium 6.0-6003

Opening a ServiceDesk Ticke with OpManager Premium 6.0-6003

When I try to create a Log a Ticket Notification Profile, the Exchange and SQL Application Monitor selections are not present. How can I open a ServiceDesk plus helpdesk ticket with OpManager 6.0 Premium build 6003 when one of the Exchange features is in a failure state?
