New Event Log rule not monitored

I have created an event log rule under applications for an event log entry that is created by an in house application.  I have associated the rule with the server it runs on and added two notification profiles (SMS and Email) to notify on the event.  When I cause the application to fail, thereby creating an event, it is not seen at all by OpManager.
