recently i have upgraded servicedesk plus msp to 10522 it is using MSSQL database

strange thing happens some times users can not see approval sent to them on "myapproval" widgets in home page , they received approval email notification also saw approval notification in notification bell on top bar, but they could not see approval on MyApproval widgets on HomePage, when i try to send approval again manually to him/her they could see it on "myapproval" widget , any one can help? 

i want to add note maybe issue related: my approvals is predefined in template, but some times when request get created we have to remove some approvers for example when Request named "CreateAccount" raised Approver1(stage1) and Approver2(stage2) and Approver3(stage3) must approve it but some times we have to remove "approver1" from the flow so first we removed "approver1,approver2,approver3" and manually add "Approver2" in stage1 and "approver3" in stage2
