How exactly does the approval system work?

Can I put in multiple approvers and either one of them can approve?  Like an OR approval.  If there are 3 approvers, only 1 is needed to approve in order for the request to get approved.

What if I want mutiple AND approvers?  Like all approvers should approve in order for the request to be approved.  If one rejects, then the whole request is rejected.
