When I monitor MS SQL Service in new monitor in app manager, I receive this message.

but when I arrive in monitor details, MS SQL service is DOWN

Host Name changes and MS SQL service is DOWN but server with the same IP (10.255.137.4) is Up.

what should I do? please help me.
