Missing Requests

Hi - wonder if anyone has any clue why this is happening...
 
For some reason when a user puts a ticket in for one specific site the tickets gets created but can't be seen anywhere. 
 
Its not in the queue for that site.  It's not in any other queue.  It just gets created and then vanishes. 
 
I have tried creating a ticket on several peoples behalf but they always go missing. 
 
The Site, Group and Department have all been set up in the same way as the others and the standard business rules have been applied with no special exceptions. 
 
Why would this be the case?  Has the site not been set up correctly?
