Issues with Self Update Layout

Good Afternoon,
 
We are trying to modify the self-update layout to include entries that are required for our business.  So far, so good, but some of the fields are not working as expected.
 
1 - We are using the Office field with a drop down pre-defined list of offices.  The entires must look like this "City, ST" (where ST is state or province).  Unfortunately, it looks like it's not showing anything before the comma.  Is there a way to format this control to allow the commas?
 
2 - In a previous tool, if we selected a specific office, the address informatino would auto-populate.  Is there a way to set this up? 
 
3 - We have the manager field, and it shows up, but we have a few situations where users' managers are in different domains.  Is there a way to tell that field to look up in another domain?  I'm assuming that might not be possible, but thought it would be good to ask if we can do it.
