I not sure if i am doing something wrong but can someone show us where this layout is set

We have created an incident template that looks like this

If we create an incident using the template it is fine when editing the request it has the same layout but once the incident has been created the layout is completely different

Does anyone know where the layout for the 2nd part is pulled from
