How to make report show comment wording as input while change become on hold status

How to make report show comment wording as input while change become on hold status

Dear Sir
 
         We really like the feature input comment when technician change status become on hold or stop timer of incident.
May I know that how to make the report can show that comment or have any field to select then show in the report?
