How to association threshold to monitor ?

How to association threshold to monitor ?

Hi : 

I add a WMI Performance Monitor .

Monitor Name : ARR Health 

Monitor object is :  Win32_PerfFormattedData_ARRCounterProvider_ApplicationRequestRoutingServer _Total_Health

Value : 0 OR 1 

And I want to create a threshold name : ARR_Health_Threshold

So How can I assign  ARR_Health_Threshold association to ARR_Health Monitor ?
