I would like to implement a script code that adds a radio button (or check box selection) with "Yes" or "No" options to the description field. This radio button should allow the user to select only one value. Please note that I do not want to add an additional field as a pick list radio. Instead, I want the radio button to be displayed within the description field on the requester page.
