How to add a custom tab to the top navigation?

How to add a custom tab to the top navigation?

Can someone point me to where the html template is located in the folder structure so I can add a custom tab in the header that points to another page? I am fluent in HTML, CSS and scripting. I just need to know where to customize the main template.
