History does not always update with all actions on a ticket

Hi,
 
We have noticed that the history tab of tickets (incidents and SR´s) do not always update with all actions that have been taken on them.
 
We have an example of one ticket that was passed to a support group on 11th August and only just passed back to a technician on the 11th December, but there is nothing in the history reflecting the change of group, or the notes added prior.  We have a screen shot of the notification alert so we can tell when it was passed back to the technician.
 
This does not happen on all our tickets, however it has happened before, but I am unable to see a pattern in why this happens in some tickets and not others.
 
My Version is 9.0 9116
