Help -- Is it possible configure "Request format"?

Hi all,

I have a situation that I need configure a template for end-users. When they open a request/ticket that these requests generates "sequences" with a format like example below:

Example: Request_Sequence_Number/Year

001/2015

Is it possible to configure like that?
