Error: "Device already present in the System" but I can't find the device in the OpManager console

Error: "Device already present in the System" but I can't find the device in the OpManager console

I am trying to add a device from Admin->Add Device, but I am getting an error message stating: Device already present in the System.  I have looked through the console but cannot find a devices with the IP address of the device I am trying to add.
 
Any idea how I can get this device to add in OpManager?
