Dynamic templates for ticket submissions?

We have a need for having certain questions asked when getting requests in for various types of tickets. Ideally, I'd like to see that when you select the category for the ticket, the problem description area is populated with a template containing various questions that should be answered for that ticket. Not wanting a form, but having the questions there to be answered by the user, or that the tech will know to ask for on any calls coming in.
For instance:
For software_1 tickets, we need:
1) Software version #
2) Menu options/screen where problem is seen
3) Box/entry field where problem is seen
4) Data entered into the field to create the problem
5) Text/error message seen
6) Request to include screen shot
For software_2 tickets, we need:
1) Software version #
2) Database being used for data
3) Menu options/screen where problem is seen
3) Customer #
4) Account #
5) Problem description
6) Text/error message seen (if applicable)
7) Request to include screen shot
For software_3 tickets, we need
1) Software version #
2) Module name
3) Problem description
