DueBy Dates on Incident Template?

DueBy Dates on Incident Template?

Maybe I'm just missing it, but I can't find how to add the DueBy Date field to an Incident Template so the requester can enter it. The only place I see to add that date is to view the request as a technician, click to edit the request and then the DueBy Date shows up.
