I am creating a new form in the Service Catalog for new employees.  We have about 30 applications an employee may or may not need access to.  On load all forms/fields are hidden except the list of possible applications the user may need access to. If employee needs access to applications 1,3, and 8, they check those applications and the form shows the additional information for those applications.

Once the form is submitted a request has been created.  In the request all the forms/fields for the other applications show up with "Not Answered".  Is there a way to eliminate the hidden fields from being included in the request?
