At the moment on our ServiceDesk we have setup many different request templates for our network shares that automatically go to the folders owner to approve. this has resulted in over 80 different templates just for folder access.

I need a way to have just one template that then when the requester picks the subcategory for the folder they need access to and submits the approval goes to the right person. 

please can you let me know if and how i can do this?
