Device says added successfully, but cannot find device

Device says added successfully, but cannot find device

I had a configured device (router) that I deleted at one point and am now trying to add back to be monitored. I add the device via IP and OpManager indicates the device was added successfully; however I cannot locate the device anywhere within Opmanager and when I search for the IP it indicates device does not exist.

Is there a problem if a device did exist, was deleted, and one tries to add again? I am on build 6000.
