The "Description" field is missing in our Default Template. I have already done the following things:

1. Reset the ManageEngine SDP Services and start again

2. Reset the server

I have run the following command in the Report Query: 

SELECT * FROM form_customizer WHERE form_name LIKE '%default%'

Which gives me the attached output. 

Please could someone let me know what to do to correct the issue.
