We've configured our default request template to make the Description field mandatory, but that doesn't mean that our users are going to supply good information while describing the problem. What we'd like to do is have the Description field prepopulated with questions for them to answer. This is after trying to create additional fields with text boxes for these questions, the inability to resize the multiline text boxes make the form unuseable and frankly ugly but SDP's god awful formatting problems is material for another post.

So here's what we need specifically. We'd like a series of questions to be pre-typed into the description area with blanks between each line for our clients to type answers. Here are our sample questions:

What is happening?


What were you doing when it happened?


Can you recreate the problem?


When did this first start happening?


Has anything been changed in your system lately (updates, new software, new hardware, etc.)?


If there is an error screen involved, are you including a screenshot of it?


Anything else you can think of?



















We tried typing these into the request template (again formatting them to look decent is a very backward process but we got them in there. What happened when we tried it? Users were able to type answers under each line, but when they hit submit the answers were removed from under the questions and then submitted in reverse order. Don't believe it, check out what we got.

If you look closely, the last answer at the bottom, is actually the response to the first question. Second to last, answer to the second question, and so on.

Any ideas on how to get a questionaire into our default request template or answers as to why our questionaire didn't work?
